**Carryout Meal Order Form** 



ALL MEALS will be FREE for children age 18 years or younger until further notice. Children do not need to be enrolled in the District to receive meals and do not need to be present during meal pick up.


Under these guidelines, meals are available on a first come, first serve basis.

It is important that you continue to register for meal pick up weekly using the Carryout Meal Order Form.


All families are encouraged to complete a Free and Reduced Meal Application to determine meal eligibility. More information on meal applications can be found here.


Carryout meal distribution will be every Monday and Wednesday. Distribution days are subject to change based on the approved school calendar.

  • Pick up locations and times:
    • 11:45 AM - 12:00 PM - West Leechburg Fireman's Recreation Hall
    • 12:00 PM - 01:00 PM - LASD Cafeteria
    • 12:15 PM - 12:30 PM - Gilpin Township Volunteer Fire Hall
    • 05:00 PM - 06:00 PM - LASD Cafeteria
  • Pick up via drive or walk up
    • If walking up, please be mindful of social distancing recommendations
    • Consumption of food is not permitted at distribution sites
  • Weekly Distribution:
      • Monday - distribution will include (2) breakfast and two (2) lunch meals
      • Wednesday - distribution will include three (3) breakfast and three (3) lunch meals
  • Pre-Order Meals:
      • Submit a Carryout Meal Order Form EACH week you wish to receive meals
      • Orders accepted 2 PM Thursday through 9 AM Monday
      • This order form will account for meal distribution on Monday and Wednesday
  • Required at pick up: Last name provided on the carryout meal order form and number of meals ordered


Pandemic Electronic Benefit Transfer
  • Per the PA Department of Education 6/25/2020:
    • Families that receive SNAP or TANF should have already had their benefits issued to their EBT card. If not, you may submit an inquiry at
    • All others, cards are in the process of being issued and mailed, but due to the very large volume, the timeline is slightly delayed. Households should receive their cards by the end of July.
