K-12 Enrollment
The Leechburg Area School District uses online forms for enrollment. Click the link to begin enrolling a student in our district.
Step 1: PRE-REGISTRATION
The first step of the registration process is "pre-registration." Once you submit your pre-registration and the pre-registration is approved, you will receive an email with an ACCESS ID for your child and instructions to complete the Enrollment Process.
Step 2: Enrollment Forms and Document Uploads
You will have to create a PowerSchool Parent Portal to complete the enrollment process. See PowerSchool Parent Portal Instructions below.
Be prepared to upload required documentation in the PowerSchool Parent Portal as part of the enrollment process. Required documents are listed below:
- Student's Birth Certificate
- Student's Current Immunization Record
- Parent's Driver's License, State ID, or Military ID
- Gas, Electric, Water, Mortgage or Lease with your current address...two (2) required for verification of address
- Custody Agreement, if applicable
- Guardianship Paperwork, if applicable
- Kindergarten Enrollment Required Exams - Medical Examination, Dental Examination, and Medical History Form
Your child is not considered enrolled until all forms are filled out and all documentation is received. Your child's start date at Leechburg will be communicated to you by the Enrollment Coordinator once you have completed enrollment forms and provided required documentation.
Medical Packet
- Physicals: Due at grades 6 and 11 (Required for Kindergarten Enrollment) Print from link below
- Dental: Due at grades 3 and 7 (Required for Kindergarten Enrollment) Print from link below
- Medical History: Kindergarten Enrollment - complete on the Parent Portal
- Medical Permission Form: For any student for the nurse's records to administer medication- complete on the Parent Portal
