Request Forms for Board Consideration

Please submit your completed forms to the Building Principal for initial approval consideration.

The deadline for School Board consideration of request forms attached below is the first Wednesday of each month by 12:00PM in the Superintendent's Office for a regularly scheduled Board Meeting (Public Work Session/Agenda Setting Meeting and Regular Monthly Board Meeting). For consideration at a Special Board Meeting, the deadline is 12:00 PM, seven (7) days prior to the meeting.

These deadlines allow for sufficient time for the School Board to review and consider your request and your cooperation is greatly appreciated:
