Immunization Regulations

 

The present school health laws of the State of Pennsylvania require that a child be immunized against the following diseases before enrollment.

  • 4 doses of DIPHTERIA/TETANUS (usually given as DTP) one dose to be given on or after the 4th birthday.
  • 3 doses POLIO vaccine. One dose to be given on or after the 4th birthday.
  • 2 properly spaced doses of MEASLES vaccine (usually given as MMR).
  • 3 properly spaced doses of HEPATITS B vaccine.
  • 1 dose of VARICELLA vaccine or proof of history of the chicken pox disease.  This vaccine is now required for all new school enterers (kindergarten or first grade) and for students entering 7th grade.

 

Listed below are the Health Service forms most commonly used.

  • Medication Permission Form:  This form should be completed when requesting medication be dispensed to your child during the school day. Please login to the Parent Portal to complete the form for your child. 
  • Physical Exam Form:  This form should be completed by your child’s Primary Health Care Provider for the following school years:
    • Entry (kindergarten or 1st grade)
    • 6th grade
    • 11th grade
  • Dental Health Form:  This form should be completed by your child’s Dentist for the following school years:
    • Entry (kindergarten or 1st grade)
    • 3rd grade
    • 7th grade
